To register for the SCOR 2011/2012 Select soccer program, follow these instructions:

1. Contact the team's head coach for sign-up permission. Contact coach list can be found here.

2. Sign-up for the program online by clicking this link and selecting a payment option: SCOR Select Program link

3. Print and fill out two forms (click on highlighted bullets):

• Player registration form
• Medical release (*requires a notarized signature)

4. Items to mail:

• These 2 forms attached, printed and filled out completely (with notary on medical form).
• A copy of the player's birth certificate (if you have not played for GVSA before).
• A 1" x 1" headshot photo with player's name on back.

5. Mail these forms to:

6. Notify the team coach that you have completed these items.
